Membership Fee for the year of 2011 會員年費
(1 Jan 2011 to 31 Dec 2011) :

Life membership 永遠會員 : HK$1,200
Ordinary membership 普通會員 : HK$120/year
Pupil midwifery membership 附屬會員 : HK$60/year

Payment 付款方法:

Please send us a crossed cheque made payable to "Hong Kong Midwives Association".Kindly state your name and contact telephone number at the back of the cheque and enclosed with a return envelope. You can send to us through the representative in your working organization or mailing to Hong Kong Midwives Association D1, 13/F., Hyde Centre, 221-226 Gloucester Road, Wanchai, Hong Kong.
(Remarks : For new joining member, please enclosed with a completed application form when sent to us.)

請以劃線支票繳付會費，收款人寫「香港助產士會」， 並在支票背面註明姓名及聯絡電話。
